ANNUAL NOTICE OF PRIVACY POLICY AND PRACTICES OF THE LEWER AGENCY, INC. AND ITS AFFILIATES

1. Personal information, for the purposes of this notice, includes any information that identifies you personally and is not otherwise available to the public. Examples of personal information include name, social security number, and address, as well as financial information such as credit history, income, financial benefits, and policy and claim information. Also included is health information such as your medical records, and other information about any illness, disability or injury.

We permit authorized employees to have access to customer information only when they need to know such information in order to provide products or services to you, or to otherwise conduct our business. Employees who have access to customer information are required to protect and keep such information confidential. 8. The Lewer Agency, Inc. will provide this notice to its clients and potential clients at the inception of our business relationship and annually thereafter. Due to the rapid changes occurring in the financial services industry, we reserve the right to revise our privacy policy as necessary. If and when amendments are made to our privacy policy, we will send you a revised notice and will post the revised notice on our website.
