Next to medical insurance, dental coverage is the single most requested benefit among employees today. Lewer HealthySmile is available with two plans, Plan 1000 and Plan 1500.
Minimum Group Size:
- 2 or more enrolled employees.
Participation Requirements:
- Groups with less than 5 full-time employees not covered under any other dental insurance, 100% of the remaining eligible employees must participate.
- Groups with 5 or more full-time employees not covered under any other dental insurance, 75% of the remaining eligible employees must participate.
Employer Contribution:
- Employer must contribute at least 50% of the employee premium.
